To expedite and save you time in the office, in addition to coming prepared to discuss your child’s need with our orthodontists, we have provided our Patient Forms link below. New patients are asked to please print off the forms and complete them before you arrive for your appointment, as it could delay the appointment and cause rescheduling. If you do not have an internet connection and/or printer at home, please arrive 30 minutes early so you can complete your paperwork prior to your appointment time.
